What is Crossroads Care ?
Crossroads Care Cheshire East service is about improving the lives of carers by giving them time to be themselves and have a break from their caring responsibilities. Our aim is to provide a reliable service, tailored to meet the individual needs of each carer and the person they are caring for.



Who can make a referral?
CCCE operates an open referral system and we are more than happy to receive calls from carers. Referrals are also received from professional sources. These include social workers, GP’s, District Nurses and other voluntary organisations, for example, the Parkinsons society and Alzheimers Society. Self referrals are also welcome.


What happens after the referral has been made?
Once a referral has been made our Care Coordinator or senior member of the team will contact the carer to arrange a suitable time to visit and discuss what services are available and appropriate.

A senior Carer Support Worker will then develop a detailed care plan and risk assessment through discussion with the carer and the person they care for. The plan will detail the care to be delivered and will be regularly reviewed.



What costs are involved?
Most of our services are free at the point of delivery. We can offer short carer breaks free. Care can also be purchased privately or through the Direct Payment Scheme.



What tasks can the Carer Support Worker carry out?
Trained Carer Support Workers will take over from the carer in order to give them a meaningful break and some time to themselves. They will carry out any tasks detailed in the care plan. In addition specialist tasks can be agreed and undertaken with training being provided by a suitable professional.



Are Short Breaks always carried out in the home?
No. Some carers appreciate time in the home whilst the person they care for enjoys an opportunity for a change of scene for example, shopping, going out for lunch or swimming supported by our Carer Support Worker. Support groups also offer a change of scene.



Are staff paid workers or volunteers?
All Crossroads Care Cheshire East, Carer Support Workers are paid staff who have undergone a rigorous induction and training programme. (see quality standards). Volunteers often help with our group support and Young Carers Project



Are all staff police checked?
All staff are CRB checked and asked to provide references. New staff are not allowed to start work until this check has been confirmed in writing and the individual has been deemed suitable.



What will happen if the Carer Support Worker is off sick or on holiday?
Crossroads Care Cheshire East will endeavour wherever possible to find a replacement CSW to cover planned delivery of service.



Will I always have the same Carer Support Worker?
It is Crossroads Care Cheshire East's philosophy to provide a regular team of staff for each individual situation. Crossroads Care Cheshire East feels this continuity is important to build up trust and commitment.



I’m a young carer. What help can you offer me?
Our Young Carers Project offers regular support groups plus the opportunity for social activities, residential trips and the chance to meet other young carers and develop friendships. We also offer one to one support. Visit our Young Carers pages for more details.
