Carer Breaks:
The range of services provided by Crossroads Care Cheshire East is about giving time to carers. Time to go shopping, meet a friend or simply get some well earned rest.
Time for themselves
We offer practical support, usually in the home, by supplying a trained Carer Support Worker (CSW) who will assume the duties of the carer for a short period of time allowing the carer to have a break.
We provide support for carers of all ages who care for people with any illness or disability including:
Adults with physical disabilities,
Older people,
Mental health problems,
Dementia,
Children with physical disabilities and
learning disabilities.
How it works
CCCE operates an open referral system. We encourage referrals from carers, families, health professionals, Social Services, voluntary organisations and many other sources.
We understand that having someone come into your home can feel intrusive so we aim to address and dispel any anxieties you may have from the beginning.
Once a referral has been received our Care Coordinator, or other member of senior staff, will arrange a visit to discuss our range of services and assess what will work best in each situation.
If the scheme is able to help, a senior care worker will work with the carer and the person they care for to put together a care plan which details exactly what care is to be delivered and when. The care plan will be reviewed annually. A risk assessment will also be undertaken at this time.
Our Support Workers
Our Carer Support Workers are carefully chosen before working in your home. They undergo a rigorous recruitment and training process and are CRB checked before they start work with CCCE. A Carer Support Worker will be sensitive to your needs and expertise as a carer.
- A regular team of CSW’s will visit each week providing a continuity of care. This also enables the CSW to become familiar with family routines and the needs of the person they are caring for.
- All CCCE Carer Support Workers receive ongoing training.
- Our CSW’s have strong leadership and support from senior staff, attend regular team meetings and have individual supervision sessions.
