How to access our Services:


Crossroads Care Cheshire East, Manchester & Tameside operates an open referral system and we are more than happy to receive enquiries.

Carers can also access services from CCCEMT through:

• Social Services
• Health Professionals
• Voluntary organisations
• Third party referral (family member or friend)
• Direct Payments and Individual Budgets
• Private clients

Once a referral has been made, the scheme will then contact you directly to arrange a mutually convenient time for our Care Coordinator or a senior member of staff to conduct an in-depth assessment with you at your home, or prior to hospital/hospice discharge, so that we can be sure we will be able to support you and meet your expectations of the service.
