Palliative Care:


Crossroads Care Palliative Care Service
The Palliative Care Team in the Cheshire East area, with support from Macmillan provide a flexible, quick response, home based service of palliative care which offers a high standard of care for those who wish to die at home.

This service is for people who have advanced terminal illnesses, who may already be at home or awaiting hospital or hospice discharge. Referrals should be made by a Health Care Professional, e.g the GP, District Nurse, Macmillan Nurse etc. However, we do encourage an open referral system and are happy to receive calls direct from carers to give advice on how to access this service.

The service is flexible and responsive to patients and their carers around the 24 hr period. It can be offered on an occasional or regular basis, for a few hours or longer periods, during the daytime, evening or overnight.

Crossroads Care Carer Support Workers are not qualified nurses but are trained to the level of a Nursing Auxillary and they receive specialist palliative care training. In certain cases extra training may be required and can be agreed and arranged between a senior member of Crossroads Care Palliative Care Team, the District Nurse, Macmillan Nurse or other relevant professional.

Bereavement Support

Losing a loved one can be devastating and often, after bereavement, the support system which has been in place throughout a loved ones illness disappears.

Your need for help and support at this time may be met by families and friends. However, many carers tell us that at this point it would be beneficial to continue to receive support for a period of time whilst coming to terms with loss and change.

This service will enable carers to continue to receive help from Crossroads Care for a short time after which they will have the option of attending a bereavement support group.

The support group will offer the opportunity to talk through problems with a trained counsellor and meet others in a similar situation.
